Write in the date and price

Use the date the vehicle actually changes hands. If it's a gift, write "$0" or "gift" instead of a price.

Add seller details

Full legal name and current address, spelled exactly as they appear on ID.

Add buyer details

Same standard: full legal name and current address, no nicknames or shorthand.

Enter the vehicle information

Year, make, model, body style, colour, and VIN, copied exactly from the registration or the VIN plate. Double-check every character.

Record the odometer reading

Read it straight off the dash at the time of sale.

Sign and date

Both parties sign in person. Two copies get completed: the buyer keeps the original, the seller keeps a copy.
